Field Crest Care Center is a municipally owned, not for profit facility, located in the rural community of Hayfield, Minnesota. It’s just minutes from Rochester, Austin, and Owatonna, and within driving distance of the Twin Cities metro area.

Since 1969, Field Crest Care Center has been offering skilled nursing care. The following decades brought two expansions, and in 2005, Field Crest began offering Assisted Living services in a remodeled wing of the facility.

Field Crest Care Center provides a wide variety of services to meet an individual’s needs, including inpatient and outpatient physical, occupational and speech therapy. Short term skilled nursing care is also available for those in need of rehabilitation services.

Photo of assisted living apartment complex in MinnesotaCrest View Villa was added in February 2000. This attached apartment complex consists of 20 units designed for independent or assisted living. One and two bedroom apartments are available and an assisted living package can be created from a menu of services.
