Field Crest Care Center is a municipally owned, not for profit facility, located in the rural community of Hayfield, Minnesota. It’s just minutes from Rochester, Austin, and Owatonna, and within driving distance of the Twin Cities metro area.

Field Crest Care Center provides a wide variety of services to meet an individual’s needs, including inpatient and outpatient physical, occupational and speech therapy. Short term skilled nursing care is also available for those in need of rehabilitation services.

Since 1969, Field Crest Care Center has been offering skilled nursing care. The following decades brought two expansions. Crest View Villa was added in February 2000. This attached apartment complex consists of 20 units designed for independent or assisted living. One and two bedroom apartments are available and an assisted living package can be created from a menu of services. In 2005, Field Crest began offering Assisted Living services in a remodeled wing of the facility.

Field Crest Care Center employs nearly 80 staff members in various departments, including Dietary, Nursing, Laundry, Housekeeping, Maintenance, Activities, Administration and Social Services. As one of the largest employers in the city of Hayfield, Field Crest offers competitive pay and an exceptional benefit package to employees.

Field Crest is affiliated with the Aging Services of Minnesota and the American Association of Homes and Services for the Aging.
