Field Crest Skilled Nursing Care
Field Crest strives to provide a welcoming atmosphere for individuals choosing a temporary or permanent home. Residents live in either single occupancy private rooms, or in double occupancy shared rooms. To create a comfortable, home-like environment, residents are encouraged to bring some of their own furniture, personal items and decorations.
Field Crest’s interdisciplinary team provides personalized, quality care to meet the individualized needs of each person. We are Medicare and Medicaid certified.
The nursing staff at Field Crest provides 24-hour skilled nursing services, and certified nursing assistants are available to aid with personal care such as bathing, grooming, dressing and eating. The nursing rehabilitation program works with residents to maintain their physical abilities and prevent loss of range of motion.
Top-quality physical, occupational, and speech therapy services are provided in-house and on an outpatient basis, by Aegis Therapies, Inc.
Resident Lifts
In addition, Field Crest Care Center is partnering with the League of Minnesota Cities to offer special ceiling-mounted, safe resident handling devices, or lifts. These lifts allow staff to easily and safely move a resident from place to place in their room and into the bathroom. There are currently 14 resident rooms and 3 bathing areas where lift devices have been installed.
Personal services
Housekeeping, laundry and other daily services are provided by the trained staff at Field Crest. Pride is taken in ensuring a clean and healthy environment, and in making sure residents’ personal clothing is laundered and returned promptly.
An independent beauty operator provides weekly services in the onsite beauty/barber shop.
Field Crest also offers a resident security system to ensure that residents with dementia do not leave the facility unescorted.
DietaryA certified dietary manager works with a consulting registered dietician to plan meals, ensuring that residents receive a balanced and interesting menu. The dietary staff will assess each individual’s personal needs, preferences and allergies, along with the doctor, to create a healthy and satisfying menu.
Social Services
Field Crest Care Center employs a licensed social worker to serve as an advocate for residents and their families, and to provide assistance for discharge planning. The social worker is available to discuss an individual’s safety upon returning home, as well as provide resources for home health services. Social services can also help families explore alternative options for those who are no longer capable of residing in their own home. Each person’s unique challenges are identified to assure their psychosocial needs are being met.
Activities
A Certified Activity Director creates a daily schedule of recreational activities. Resident needs and preferences are an important part of the planning process, and they can choose to participate in structured large group activities, small informal group activities, or independent activities.
Area clergy support Field Crest with visits, worship and communion services, providing multi-denominational spiritual care. Sunday morning services are provided in a comfortable community room. The facility also offers social areas where residents can gather with their families and friends.
Periodic social outings allow for continued involvement in the community, and special events and parties are regularly scheduled to provide opportunities for socialization and fun.
Hospice ServicesFor those residents who are in the final stages of life, a comfortable, nicely furnished family/hospice room is available. In addition to the private resident room, there is a separate sitting area where family members can gather and spend time together. Field Crest also contracts with several hospice providers in the area to offer choices of hospice services to the resident and their families.
Field Crest Assisted Living
The assisted living apartments at Field Crest provide the resident with privacy and independence while giving them the support and assistance they need to stay as independent as possible. Assisted living residents are welcome to participate in all activities of Field Crest Skilled Nursing Care that interest them, including regular outings and special events. A nurse-call system and 24-hour nursing supervision provide safety and security. Services such as housekeeping and laundry, three meals a day, medication administration, and assistance with bathing are available for additional fees. Field Crest Assisted Living is located inside Field Crest Care Center, and a separate, secure entrance provides convenient access for visiting friends and relatives.
Crest View Villa
Residents of Crest View Villa live in one or two bedroom apartments. These lovely units include living room spaces and full kitchen facilities, separate bedrooms and individually controlled heating and air conditioning. An emergency call system in each apartment provides peace of mind in case of falls or medical emergencies. Each apartment also features a walk-in/roll-in shower with seat and adjustable shower wand.
Continental breakfast, transportation scheduling, and all utilities (except telephone and cable television) are included in the monthly rate. Other amenities include a common dining room with kitchen, a free, fully equipped laundry room, handrails in the hallways, a comfortable social room and a cozy fireside nook. For an additional fee, other services including laundry, lunch and/or supper, light housekeeping, medication administration, assistance with bathing and some care services are also available.
Special activities are planned regularly for tenants, and outings and shopping trips allow residents to remain active in the community as they acquire needed items.
Crest View Villa is attached to Field Crest via a walkway, which makes visiting friends or loved ones in the nursing home much easier and more convenient. This also allows tenants to take part in the special events and worship services that are scheduled at the care center.
